Evaluation of changes in statographic parameters and muscle strength in patients with sacroiliac joint dysfunction after special exercises

Abstract


Background. Sacroiliac joint (SIJ) dysfunction is an impaired ability of SIJ to transfer the vertical loads throughout “spine — sacrum — pelvis” system. It is associated with violation of mobility and support ability of SIJ. Authors developed physical exercises to improve “spine — sacrum — pelvis” system stability and symmetrical SIJ mobility. The purpose was to investigate effectiveness of the physical exercises in patients with SIJ dysfunction. Materials and methods. Twenty three patients with SIJ dysfunction were examined. Inclusion criteria’s were: pain in the area of spina iliaca posterior superior irradiated to groin, buttocks, hip; more than 3-month history of pain; failure of the previous conservative treatment; positive 4 to 8 and more provocative tests. The exclusion criterion was a positive result of only 1 or 2 of provocative tests. Patients’ age was 18 to 66 years. Average age was 41.8 years. There were two sets of physical exercises. The goal of the first one was to recover symmetrical muscular spinal and SIJ stability and pelvic symmetries. The goal of the second one was to recover muscle strength and endurance. These muscle were m.piriformis, m.gluteus maximus, m.gluteus medius, m.tensor fasciae latae, m.erector spinae, m.biceps femoris, m.iliopsoas, m.transverses abdominis, m.obliqus abdominis externus, internus, m.rectus femoris, m.quadrates lumborum. Patients preformed these physical exercises one time a day during two weeks. Patients were examined before and after treatments. Patients were examined on a force-platform. Result. All patients had a decrease of pain. They have more symmetrical leg loading in two-leg standing and normalization of the centre of gravity. Muscle balance was recovered. Kext/flex of all patients improved to 1.3 ± 0.1. All patients have positive dynamic of provocative tests. Conclusions. Technologies of symmetrical and selective physical exercises are very effective in the treatment of patients with SIJ dysfunction.
