Treatment Outcomes in Patients with the Proximal Humerus Fractures

Abstract


82 patients with the proximal humerus fractures were examined a year and 5 years after injury. The study group included 42 (51.21 %) patients who underwent osteosynthesis for the proximal humerus fractures using plates with angular stability. The control group consisted of 40 (48.79 %) patients treated conservatively (immobilization and drug therapy). The evaluation was carried out by means of Oxford Shoulder Score and Constant Shoulder Score, stages of osteoarthritis of the shoulder joint — by J. Kellgren. A year after the injury, according to Oxford Shoulder Score, the average treatment outcomes of patients in the study group were significantly better than in the control group — 38 and 28 points, respectively, the same situation persisted 5 years after injury. By Constant scale, a year after the injury there were received 30.5 % of excellent and good results in the study group, 20.72 % of excellent and good results in the control group. 5 years after the injury, the number of excellent and good results in patients from the study group was 32.92 %, the number of excellent and good results in patients of the control group — 19.49 %.
5 years after the injury, in the study group the number of patients without secondary osteoarthritis or with I–II stages of osteoarthritis of the shoulder joint was 39.02 %, in the control group most patients had severe osteoarthritis of the shoulder joint (stages III–IV arthrosis in 39.04 % of patients). It is found that contracture of the shoulder joint has a significant impact on the development of osteoarthritis of the shoulder joint.
